Hope Home Care Earns High Ratings

Dayton, OH, August 5, 2015: Hope Home Care has been rated a 4.5 (out of a possible five) “Quality of Patient Care Star Rating” from the Centers for Medicare & Medicaid (CMS). The ratings are available on Home Health Compare which was created by CMS on Medicare.gov to provide quality of care information about Medicare-certified home health agencies. Ratings are calculated based on 3 process measures and 6 outcome measures:

Process Measures:

  1. Timely Initiation of Care
  2. Drug Education on all Medications Provided to Patient/Caregiver
  3. Influenza Immunization Received for Current Flu Season
  4. Outcome measures:

  5. Improvement in Ambulation
  6. Improvement in Bed Transferring
  7. Improvement in Bathing
  8. Improvement in Pain Interfering With Activity
  9. Improvement in Shortness of Breath
  10. Acute Care Hospitalization
